Team1Medical and a non-profit organization in Fort Worth are pursuing an Options Counselor to join the team! The Options Counselor will provide social reassurance calls to consumers and families that have secured assistance with a specific agency. The Options Counselor will also participate in outreach events to reach people in the community needing assistance. Options Counselor’s Responsibilities
- Conduct access and assistance interactions with community consumers over the phone or face-to-face.
- Contact people in the community and provide COVID-19 education support information.
- Maintain a current list of locations where consumers can access vaccines
- Accurately enter all contact information into the client management information system and client files
- Maintain confidentiality of client information as required by State and Federal laws, regulations, and professional practice standards
- Perform other job duties or responsibilities as requested or assigned
Options Counselor’s Qualifications
- Bachelor’s degree in Social, Behavioral, Health or Human Services is required.
- Case Management and Customer Service experience are required
- Professional phone etiquette is highly preferred
- Must be comfortable with traveling across the county
- A Valid Texas Driver’s License is required
- Must be able to pass a pre-employment drug screening and criminal background check
